Getting Started
Waiver Types and Programs
Virginia offers three main waivers to support individuals with developmental disabilities:
- The Building Independence (BI) Waiver for adults who can live mostly independently
- The Family & Individual Support (FIS) Waiver for those needing varying levels of support
- The Community Living (CL) Waiver for individuals requiring 24/7 assistance, often in group home settings.
Intake Paperwork
The journey to receiving personalized support begins with our Intake Paperwork, which includes the Application for Admission and Preliminary Assessment. This essential step allows us to gather detailed information about the individual’s needs, preferences, and goals. By completing this paperwork, families provide us with the foundation to create a tailored plan of care that aligns with the individual’s unique strengths and challenges.

Annual Paperwork
At our organization, we prioritize the rights and dignity of every individual we serve. As part of the annual paperwork process, families and individuals are required to complete the Individual Human and Home and Community-Based Services (HCBS) Rights Acknowledgement Form. This form ensures that all participants understand their rights to privacy, respect, and autonomy while receiving services. By signing this document, individuals and their families affirm their awareness of these rights and our commitment to upholding them in every aspect of care. This annual process is a cornerstone of maintaining transparency and fostering trust between our organization and the community we serve.
Surveys
Surveys are often used to assess the ongoing needs and satisfaction of individuals receiving services. Completing these surveys helps ensure that the support provided is effective and meets the individual’s needs.
