Getting Started & Goals

We develop goals with your loved one and family. We balance what’s important to the person (their wishes—skills, friendships, employment, independence) and what’s important for the person (health, safety, stability such as medication management, safety awareness, and medical care).

Through the Individual Support Plan (ISP)—a written plan capturing the person’s priorities and their health/safety needs.

The Supports Intensity Scale (SIS) is a Virginia‑approved assessment that measures the level of support needed in daily living, community involvement, relationships, and health/safety. It guides staffing and support intensity.

Services & Staffing

We specialize in in‑home support, life skills training, and health assistance tailored for individuals with intellectual disabilities.

Your first clinical contact is with our Intake Specialists (also serving as Entry Level Supervisors) during or just after intake.

  • Intake Specialists provide a minimum of 10 hours weekly during engagement.
  • They complete a comprehensive assessment of the individual and family to define the skills staff will need.
  • We share this assessment with HR, who prescreen and identify qualified candidates.
  • The process is refined and repeated until we match you with the best‑fit staff for consistent, dedicated support.

Care Philosophy

A structured, ongoing process—often via the ISP/person‑centered plan—to tailor services to the person’s unique needs, strengths, and preferences.

Sponsored Residential Services (for prospective sponsors)

You must partner with a DBHDS‑licensed Sponsored Residential Services provider agency—the agency recruits, trains, monitors, and handles Medicaid billing, documentation, and compliance. Start by contacting your local CSB or reviewing the DBHDS provider list.

  • Background checks: state & national criminal checks, CPS registry, DMV record (if transporting)
  • Home inspection to ensure safety, accessibility, and space requirements
  • CPR/First Aid
  • Medication Administration (MAT) if assisting with meds

After a home study and approval, the agency matches you with an individual based on compatibility (age, gender, support needs, lifestyle, preferences). You’ll meet and plan a gradual transition into your home.

  • Provide 24‑hour support and supervision in a family‑like environment
  • Follow the person’s Person‑Centered Plan (PCP)
  • Collaborate with the provider agency and support team
  • Document daily supports and progress (logs, reports, incidents)
  • Attend ongoing training and sponsor support meetings
  • Remain under provider and DBHDS oversight

Sponsors receive a stipend (room & board) plus service payments billed via Medicaid by the provider agency. Rates vary by support level (often informed by the SIS).

Waiver Services in Virginia

  • Contact your local Community Services Board (CSB), the single point of entry for DD Waiver services.
  • Complete intake and screening, including eligibility review and the Virginia Individual Developmental Disabilities Eligibility Survey (VIDES).

Eligible individuals are placed on the statewide DD Waiver waitlist. Priority is set by slot assignment (Priority 1 indicates the most urgent need). The waitlist is managed by DBHDS and the CSB.

Slots are limited; many individuals wait—sometimes years. The CSB will contact you when a slot becomes available and you are next by priority.

  • Community Living Waiver: residential support (group home, sponsored residential, supported living) plus day/community services
  • Family & Individual Supports (FIS) Waiver: for those living with family or independently; supports include personal care, respite, day programs
  • Building Independence (BI) Waiver: for adults with mild support needs focused on independent living

You’ll work with your CSB Support Coordinator to create a person‑centered ISP identifying desired services (e.g., sponsored residential, day support, in‑home services). The plan is submitted to DMAS (Virginia Medicaid) for authorization.

After Medicaid authorization and provider selection, services begin once providers are in place and all approvals are complete.
